1) Clients who wish to obtain a copy of their Mental Health Records please submit a request in writing and allow 15 business days for her to complete your request. 

2) Any client who wishes to file a compliant about their mental health therapist please do so in accordance with the rule 884.2. and with the Standardized Complaint Form. All complaints must be submitted on the Council-approved complaint form. The complaint form shall be obtained free of charge from the Council's website or by requesting a copy from the Council. Please see the link above. You may also file a Consumer Complaint online with the Office of Attorney General’s Consumer Protection.
